ReForm Health Cancellation policy:
All appointment bookings (individual and group) with ReForm Health our bound by the terms and conditions of the clinic’s cancellation policy, which states:
- For cancellation or rescheduling of appointments, ReForm Health requires at least 24 hours notice prior to the start time of the appointment that is being canceled or rescheduled. Any notice given within 24 hours of the appointment start time will incur a fee of 50% of the full fee for that appointment.
- Failure to show for your appointment without any prior notice will result in a fee of 50% of the full fee for that appointment.
- If you are ill or have to provide care for someone who is ill and cannot attend your appointment, ReForm Health may waive the fee if appropriate evidence is provided (such as a medical certificate). There are no fee waivers for this circumstance if you do not notify the clinic of your appointment cancellation or reschedule prior to the appointment start time.
ReForm Health offers the option of providing appointment reminders via SMS and/or email prior to appointment start times. However, it is ultimately the responsibility of the patient/client or guardian of the patient/client to manage their appointment time/s and adhere to the cancellation policy outlined above.
Cancellations or rescheduling appointments can be made via:
- Calling the clinic on (03) 8390 8545. There is the option to leave a voicemail if the call is not answered.
- Replying ‘No’ to your SMS reminder.
